The vehicle kill medal that you use in your pack is actually the Reload This medal. The different splatter spree/vehicular manslaughter medals are probably a discrepancy similar to above, B.Net must have had a different online medal than what was provided ingame

I'm not sure of the lower quality as these medals are extracted directly from the Reach game files (could be that B.Net had higher res medals, but the difference in quality is marginal enough to not notice while playing H:O) The medals I used were scaled down (they were larger in the game files) to be almost exactly the size of the default medals as to avoid any inconsistencies in display size Good catch on the Running Riot medal, I put in the third tier assist streak medal by mistake.
